# Disability Support Services in Auburn

Auburn, one of Sydney's most culturally diverse suburbs and close to Sydney Olympic Park, is about 10km from our Revesby office. We work with Auburn families across a range of language backgrounds and connect participants with interpreters where needed.

## Why families in Auburn choose CDC

We know every participant personally

We keep our participant numbers small on purpose, so every plan is genuinely tailored to you and you're never just a case number.

Direct access to our founder

Speak directly with Azzam Ali, not a call centre. Questions get answered by someone who actually knows your plan.

NSW-wide support team

Support workers placed near you across New South Wales, who understand the local area and can respond quickly.

Clear, honest communication

No jargon, no surprises. We keep you clearly informed every step of the way, with interpreters available if you need one.

“Thank you for the service, everyone was very helpful and the homecare ladies were very good at their job.”Participant Family

## Frequently asked questions

What's the difference between plan-managed and self-managed?

Plan-managed means a registered plan manager pays your providers on your behalf using your NDIS funding. Self-managed means you pay providers directly and claim it back yourself. We work with participants under both arrangements, as well as NDIA-managed plans.

How do I make a referral or start an intake?

Use the "Start Your Intake" or "Make a Referral" button on this site, or call us directly. We'll ask a few questions about the participant's needs and current NDIS plan to see how we can help.

What happens after I contact you?

We'll arrange a short conversation to understand your goals and support needs, confirm funding arrangements with your plan manager or the NDIA, and agree on a service agreement before support begins.

Which areas do you service?

We support participants right across New South Wales, with our core team based in Sydney. Get in touch and we'll confirm availability in your area.
